Who can make a referral?
Anyone can refer. Physicians, discharge planners, social workers, home health agencies, nursing facilities, family members, caregivers, clergy, and patients themselves are all welcome to reach out. If you believe someone could benefit from our care, we encourage you to contact us.
When is the right time to refer?
Earlier than most people think. Patients and families who access care sooner experience greater comfort, fewer hospitalizations, and more meaningful time together. If your patient has a serious, life-limiting illness and curative treatment is no longer the focus, it may be time to reach out.
What information do I need to make a referral?
Basic patient information is all that is needed to get started. Our team will work with you to gather any additional clinical details needed to complete the intake process.
What services are available through a referral?
Referrals can be made for hospice care and personal care services. If you are unsure which service is the right fit, our team is happy to help you determine the best level of support for your patient or loved one.
Does the patient need to be enrolled in Medicare to receive hospice care?
No. While Medicare does cover hospice care for eligible patients, we serve patients regardless of their insurance status or ability to pay.
What happens after a referral is submitted?
