LICENSED PRACTICAL NURSE

JOB DESCRIPTION: LICENSED PRACTICAL NURSE

Provides nursing services in the patient’s home in accordance with agency standards and applicable laws and regulations. Ensures that patients receive the highest possible quality of care.

Responsible for assessment and care of patients in accordance with Hospice and HomeCare of the Valley policies and procedures. Responsible to accepting duties and reporting to the Registered Nurse with accordance to the agencies policies and procedures.

QUALIFICATIONS/FUNCTIONS:
1. Graduate of an approved school of practical/vocational nursing, currently licensed as a Licensed Practical Nurse in the State of Colorado
2. Car with required proof of insurance coverage and a Colorado Driver’s license.
3. Two (2) years of experience in home health care or related health field.
4. Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation.
5. Able to assess and communicate with ill patients, co-workers, and general public
6. Ensures quality and safe delivery of hospice and home health services.
7. Implements current nursing practice.
8. Fulfills additional performance responsibilities as assigned and as necessary.
9. Promotes effective written/verbal communication.
10. Provides information to patients regarding equipment and supply needs of the hospice and home health agency.
11. Uses equipment and supplies effectively and efficiently.
12. Fulfill duty of LPN per position functions as assigned.
13. Complies with hospice and home health agency policies and procedures.
14. Promotes education for home health care staff members.
15. Promotes education for clients, client families, and the community.
16. Participates in personal professional growth and development.
17. Completes all required documentation in accordance with the standards of the agency, and regulatory requirements.
18. Works effectively with the medical staff, other members of the health care team and administrative personnel. Maintains a cooperative working relationship.
19. Demonstrated ability to provide effective support during patient/family visits.
20. Obtains a minimum of twelve (12) hours of continuing education annually.

Physical demands and working conditions:
1. Able to work up to forty (40) hours per week.
2. Able to bend and stand an average of six (6) hours per day.
3. Able to lift up to 50 – 75 pounds.
4. Able to write up to three (3) hours per day.
5. Able to work in a stressful environment.
6. Able to drive 300 miles per day.
7. Physical demands such as walking, step climbing, and flex hours.
