During hospice care, usually a family member serves as the primary caregiver and, when appropriate, helps make decisions for the terminally ill individual.
Our interdisciplinary team consists of the patient’s personal physician, hospice medical director, nurse, home health aides, social workers, bereavement counselors, chaplain, if requested, or other spiritual counselors, trained volunteers, and speech, physical, and occupational therapists, if needed.
Our hospice staff is on-call 24 hours a day, seven days a week. The hospice care team makes regular visits for assessments and provides additional care or other services, if needed. We also admit patients over the weekend, if necessary. The hospice team develops a care plan that meets each patient’s individual needs for pain management and symptom control.
