Job Description: Hospice Director

Reports To:  Executive Director

Department: Clinical

Position Summary: Implement delivery, coordination and monitoring of appropriate services for our patients, families and caregivers.  As a member of the interdisciplinary team, the Director of Hospice will participate in the development and implementation of the patient care plan.  Additionally, the Director of Hospice is responsible for directing all hospice services with the highest professional standards in accordance with agency policies, procedures, philosophy and mission statements

Position Qualifications:

  • RN with extensive Hospice Knowledge
  • Experience: Minimum two years’ experience with hospice or professional equivilant. Management or Supervisory experience.
  • Training: Sufficient background knowledge and expertise in clinical decision-making for the patient population assigned to meet the needs of the patients.
  • License/Certification: Current RN. Current BLS required
  • Computer literacy
  • Knowledge of hospice federal and state regulations
  • Knowledge of Medicare/Medicaid hospice benefits
  • Intermediate typing skills
  • Knowledge of office machines
  • Excellent phone skills with an emphasis on customer service
  • Organizational skills

Duties:

  • Direct and supervise all hospice services and personnel
  • Participate in the development, implementation and evaluation of written policies and procedures
  • Participate in budgeting for patient care services and monitoring of expenses
  • Participate in collection and analysis of data for continuous quality improvement activities
  • Demonstrate knowledge and skill necessary to direct the provision of care based on physical, educational and safety related criteria, appropriate to the age of the patients served
  • Responsible for staffing productivity and identifies staffing needs to insure adequate staff coverage to meet safe and timely patient care delivery.
  • Provides leadership to hospice team – directs staff meetings, team huddles, and patient care conferences on a regular basis.  Assures, promotes, education, competency, orientation. Executes timeliness of all education.
  • Is knowledgeable of federal regulations, state licensing regulations and accreditation standards and is able to interpret regulations for clinical staff to insure patient advocacy, informed decision making and compliance with policies and regulations.
  • Is knowledgeable of reimbursement requirements for all third party payors.
  • Builds relationship (s) with referral sources.
  • Reviews treatment plans to insure orders, frequencies and documentation are complete and accurate. Insures all documentation is received and reviewed timely.
  • Responsible to determine acceptance of patients involves physician’s orders are followed, frequency and duration are completed by competent staff, and communication occurs between disciplines.
  • Responsible to schedule team meetings and case conferences as indicated to assure staff complete POC’s and modify orders as indicated. Team meetings may also involve update of policies and procedures and will be supervised by the director.
  • Oversight of specific management reports for timeliness of recertifications, compliance with managed care authorizations, utilization, and submission of paperwork, accuracy and completeness of scheduling, quality of care, reputation and patient satisfaction.
  • Assures that staff chart timely.
  • Collaborates with billing to ensure timeliness and clinical accuracy.
  • Is a good steward of resources working within Budget, managing productivity and practicing cost containment.
  • Contribute input for Policy and Procedure design, change, etc.
  • Oversight of utilization, electronic documentation accuracy and completeness of scheduling.
  • Annually completes Compliance Education and understands applicable policies.
  • Improve process flow throughout the system
  • Improve communication throughout all levels of the patient experience
  • Build new programs and community services

Physical Demands: Physical exertion—walking, lifting, pulling, computer work

Environmental Conditions: Exposed to products used in the office, i.e, printers, computers, dust, etc.

Working Conditions: Periods of stress—variable workloads and deadlines
