Field Supervisor

Job Description: Clinical Field Supervisor

Reports To:  Home Health Director

Department: Clinical

Position Summary: The Field Supervisor will provide clinical guidance and training to clinical staff in the field and will evaluation their performance in the field. These supervisors are also responsible for clinician/patient scheduling

Position Qualifications:

  • RN or PT with extensive Home Health and/or Hospice knowledge
  • Experience: Minimum one year experience with home health and/or hospice or professional equivalent. Management or Supervisory experience preferred.
  • License/Certification: Current RN or PT. Current BLS required
  • Demonstrates use of basic office equipment: computer/laptop , copy machine, fax machine.
  • Capacity to learn and effectively utilize software used by HCHOTV.
  • Manages a flexible work schedule (including weekends) in accordance with company attendance and reporting policies.
  • Maintains access to reliable vehicle and has a willingness to travel.

Duties:

  • Provides direction, support and motivation to subordinate employees to work towards a common goal.
  • Counsels on work related activities in an effort to increase productivity and also for the purpose of development.
  • Supervises field performance of clinical staff (nurses and therapists)
  • Understands and complies HCHOTV rules, policies and procedures and trains employees, including contract employees, to achieve the same.
  • Completes, maintains, and submits reports and records documentation to the Home Health or Hospice Director on a consistent and as needed basis.
  • Conducts informal and formal written performance appraisals on subordinate employees after the probationary period, annually, and as required.
  • Provides guidance to clinical staff on providing patient care in adhere to the professional standards of community nursing practices.
  • Maintains and adheres to HCHOTV procedures and patient care policy manuals.
  • Participates in the on-call schedule.
  • Carries a patient load
  • Complies with accepted professional standards and principles.
  • Promotes customer service orientation to all agency personnel.
  • Performs other duties and activities as delegated by Home Health and Hospice Director
  • Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term goals, in setting priorities, and in developing plan of care.

Contacts:  Families, volunteers, staff members, hospital/facility personnel, the general public.

Physical Demands: Physical exertion—walking, lifting, pulling, computer work

Environmental Conditions: Exposed to products used in the office, i.e, printers, computers, dust, etc.

Working Conditions: Periods of stress—variable workloads and deadlines
