Job Description: Speech Language Pathologist
REPORTING RELATIONSHIP: Therapy Supervisor
POSITION SUMMARY
The Speech Language Pathologist (SP) at the request of the Registered Nurse/Case Manager provides evaluation and therapy services to patients who have speech and swallowing difficulties and as ordered in the patients Plan of Care. The SP provides inservice education and consultation to the Interdisciplinary Group (IDG).
Responsibilities:
Patient Care:
1. Conducts diagnostic speech evaluations and evaluates swallowing difficulties.
2. Recommends therapeutic and functional goals for the patient.
3. Implements speech therapies when ordered in the Plan of Care.
4. Provides telephone report to the RN Case Manager post visit.
5. Documents evaluation, discussion and recommendations given.
6. Provides inservice education and consultation to the IDG as requested.
7. Attends IDG on request.
8. Maintains state required continuing education and licensing certification.
9. Completes required visits within three (3) days of request.
10. Provides telephone report to PCM within twenty four (24) hours of visit.
11. Documents visit according to policies and procedures.
General and Administrative:
1. Documents professionally, accurately, timely and according to procedure.
2. Submits documentation to the RN Case Manager, and/or accesses EMR and enters documentation on a timely basis. Submits documentation of visit within three (3) days of visit.
EDUCATION/QUALIFICATION
1. Certificate of Clinical Competency in Speech from the American Speech & Hearing Association.
2. Has completed necessary work and educational requirements for certification and is in the process of accumulating necessary supervised work experience required for certification.
3. Current state license in Colorado.
Contacts: Patients, families, volunteers, staff members, hospital/facility personnel, the general public and household pets.
Physical Demands: Physical exertion—walking, lifting, pulling, driving, step climbing, flex hours and travel.
Environmental Conditions: Exposed to infections, communicable diseases, odors, blood, excreta and hazardous materials.
Working Conditions: Periods of stress—variable workloads and deadlines. Light office work. Some evening work will be required. Exposed to terminally ill patients and their families.
